Location: Ohio
Start Date: Early December
Schedule: 7a - 5p or 8a - 6p shifts for 4 days per week
Responsibilities:
• Conduct routine check-ups and exams
• Manage hypertension cases
• Provide care for diabetes patients (Type 1 and Type 2)
• Manage congestive heart failure (CHF) cases
• Treat chronic obstructive pulmonary disease (COPD)
• Focus on ongoing management and care of chronic conditions
