Responsibilities and Duties:
• Conduct monthly visits with I-SNP patients in nursing homes
• Perform risk assessments and develop care plans for patients
• Carry out comprehensive visits for ACO patients
• Utilize GEHRIMED and PointClickCare electronic health record systems for documentation
• Collaborate with nursing home staff and other healthcare professionals
• Ensure high-quality care for patients on special Medicare plans
• Adhere to all clinic and nursing home policies and procedures
• Maintain up-to-date knowledge of geriatric care and I-SNP guidelines
• Communicate effectively with patients, families, and healthcare team members
• Participate in care coordination and care plan reviews
• Monitor and manage chronic conditions in the elderly population
• Provide patient and family education on health maintenance and disease prevention
• Contribute to quality improvement initiatives for I-SNP and ACO programs
Requirements:
• Active Ohio state license as a Nurse Practitioner
• Family Practice certification
• Experience in geriatric care and working with nursing home patients
• Familiarity with I-SNP and ACO programs
• Proficiency in GEHRIMED and PointClickCare electronic health record systems
• Excellent communication and interpersonal skills
