Job Description

We are seeking a licensed Internal Medicine Nurse Practitioner in Illinois for a locum tenens position in home healthcare. This 135-day assignment begins on 05/19/2025, with a Monday to Friday work schedule from 8:00 AM to 4:30 PM. The NP will be responsible for providing care to a diverse range of critically ill patients, including neurological cases, liver cancer patients, and those with chronic pain or severe illnesses. Key responsibilities include evaluating patient status, checking vitals, managing medications, writing prescriptions, and determining eligibility for hospice care.

Responsibilities and Duties: 

• Provide home healthcare services to critically ill patients with various conditions 

• Evaluate patient status and perform comprehensive assessments 

• Check vital signs and monitor overall health conditions 

• Manage medications, including refills and writing new prescriptions 

• Conduct recertifications for ongoing care 

• Assess patients for potential hospice care eligibility during each visit 

• Treat and manage complex cases, including neurological and cancer patients 

• Provide care for patients with chronic pain or severe illnesses 

• Typically see patients twice a month 

• Utilize Aprima software for patient management and documentation 

• Collaborate with other healthcare professionals to ensure optimal patient care 

• Educate patients and families on disease management and treatment plans


Additional Information: 

• Must be licensed to practice as a nurse practitioner in Illinois 

• Comfortable working independently in a home healthcare setting 

• Strong clinical skills and ability to manage critically ill patients 

• Excellent communication skills for effective patient and family interactions 

• Flexibility to adapt to various patient needs and home environments 

• Proficiency in Aprima software preferred
